Report: Smoke in cabin of Boeing 737 at Kerry, Ireland caused by runway de-icer

The Air Accident Investigation Unit Ireland (AAIU) released their investigation report regarding the December 2010 incident involving a Boeing 737-800 at Kerry Airport, Ireland. Runway de-icer was blamed for smoke inside the cabin and cockpit of the plane.

ANSV calls for review of Italian approach charts

The Italian investigation board ANSV is recommending a review of certain approach charts of Italian airports. The investigation into an Airbus A319 landing accident at Palermo learned that the crew used charts that were not fully compliant with ICAO principles.

The Australian Transportation Safety Bureau (ATSB) published the final report of their investigation into the fatal Embraer 120 loss of control accident at Darwin, Australia. The airplane crashed during a simulated engine failure on takeoff.

NTSB: re-evaluate crashworthiness test requirements for overhead units and bins

The U.S. NTSB notes that there have been several survivable accidents in the last 3 years in which overhead bins and passenger service units (PSU) on Boeing 737 airplanes became separated from their attachments during the accident sequences. This suggests that current crashworthiness test requirements do not provide an adequate basis on which to evaluate how these items will withstand impact forces during survivable accidents.

Report: bounces and improper control inputs cause MD-11F landing accident

The Saudi Arabian General Authority of Civil Aviation (GACA) has released the final report of their investigation into the July 2010 landing accident involving an MD-11F at Riyadh. The report concludes that the flight crew responded to bounces on landing by using exaggerated control inputs.

Report: B737 off runway in Spain following unstabilized approach and high-speed landing
